THEY were wrong wrong wrong! A doc does not have to sign it. It needs to be constructed properly, signed, notarized, and the facility needs a copy - not the original. Mine and my wife's, along with our healthcare power of attorney forms were prepared by an attorney. We have access to them in the cloud by any electronic device including a smart phone. Please don't accept erroneous information at face value. Some workers don't even know what they are. If you don't have a DNR form the hospital can provide one and provide a notary and witness for your use. And it does not have to be on yellow paper.
