I would (and have on several occasions) approached them to see if they needed any help. Even if it "looked suspicious" I would still ask, maybe from a few steps away depending on appearances. I would not hesitate to call 911 if I felt something was not right.
And I think most people do the same. At minimum take some pictures, cause you never know.
With all that said, I feel there are very few cart accessible locations in TV that present any real threat to cart theft. I know it happens, but with 45,000 carts in use (and that is conservative) even one theft a day is almost negligible.
Our two carts cost us a lot more than we paid to build our first new home so we made sure we have adequate/proper cart insurance coverage. I think most cart owners have done the same.
