Autoshow, I can tell you from my experience only but have no knowledge of how this officially works, but when I had an emergency, the paramedics explained to me that they evaluate the appropriate ER/hospital based on how crowded they are at the moment, if they have a bed available and if the ER docs can review your case in an appropriate time frame. So I agree with Barb and bargee, it depends on the level of emergency, which I very much appreciated. And, luckily, I was happy that the hospital I preferred was available for me. Everything worked out well even though it was not close to my home.
