I use a short leash and use the basket to loop the leash through. Then hook it to my dog's harness. He can move around a little but not enough to fall out of the cart. Over time he has learned his boundaries and usually just sits there enjoying the breeze.

I should note my dog weighs 16 pounds. For a heavier dog I would use a support post or a structural piece of the cart.
