Interesting question, since the letter I received had specific user ID numbers to be entered. Almost always avoid any surveys that I think are traceable to the end user (aka me). In this case I will look very carefully at the intro text to see if the data and comments are made anonymous. As someone else pointed out, and as I recall there are some very personal questions. Will need to look at these, and if these are too personal, I will opt out....
I have a general case of paranoia, and with a very few exceptions, I tend to keep a low profile on the web, and share very little personal info. As I recall last year, I started the survey, and found the questions very intrusive, so I stopped.
Hopefully this year less intrusive..............
