There were no tests, no reasons other than empirical evidence. I don't believe that this has ever been the case with any other disease in history.
Again, I don't want to get into an argument over whether homosexuality is a disease or not. I'm simply stating the facts and noting that it was mostly political pressure that caused the removal.

Being gay seems to be biologically driven and is somehow developed in childhood. That is just from my experiences around gay/lesbian people. It is not really chosen.
It has as much to do with mental illness as people liking blondes with Kentucky accents.
