Filled ours out. You do need to remember birthdays and ethnic backgrounds.
Of course, I remembered 2-24 for mine and also got my parents' right as well. I did double-check them after fact though.
Put me down as German and Irish-English-Scotch.
Dad's side came from Germany around 1870 and Mom's side from Ireland-Scotland-England around 1670 and settled in North Carolina or Massachusetts.
