When we designed our house we didn't like the selection of the light fixtures offered by The Villages for the dining room, foyer, and over the island in the kitchen. So we had the builder run wires to the electrical boxes in the ceiling where the lights would go. Then after we moved in, we found lights we liked at Lowes and installed them ourselves. There are some locations where the builder is required by code to install lights when they build the house, like over the bathroom vanities.
I suspect that you can specify that the builder not install the fans, but I'm not 100% positive about that.
