How about subacute sclerosing pan encephalitis from measles vaccine or acute idiopathic demyelinating polyradiculopathy from influenza vaccines or generalized encephalitis from smallpox vaccine or severe allergic reactions from just about any vaccine.
That being said, this appears to be a very safe vaccine and everybody should get it, per CDC recommendations. You have a much greater chance of dying while driving to the vaccine site than you do of dying from the vaccine, which in turn is much lower than if you contract the disease.
