T.J. Maxx foolishly used WEP for their WiFi security, long after WEP could easily be hacked in less than one minute, by a hacker in their parking lot. As a result of the management incompetence and reckless disregard for IT security, thousands of customers credit card information was stolen by the hacker.
If anyone is fired, it probably would be the general IT workers, instead of the incompetent and reckless management, who are responsible for the security breach that could have been easily prevented.

Well, they were warned of the exploit last year by the government cybersecurity people. Someone made the decision NOT to upgrade and/or update.
I am in government computer work right now. I actually did all my updates department wide yesterday. So someone ignored a recommendation.
Microsoft Outlook is now external through OWA - not hosted at the site. It's been over 2 years since those systems were implemented, so my guess is that they may not have updated anything since about 2018.
