I blame the Electronic Medical Records that are now pretty much required... If you don't have copious amounts of garbage in your notes, the insurance companies deny payment...

I'm dealing with $8K worth of denied claims from Medicare right now. They said it's because my notes didn't include things like "The patient tolerated the procedure well"...

Total insanity...

No one writes notes anymore, they just "check off boxes"...
