Maybe in the same place where the words have created an expectation that a person must be treated in a hospital whether or not he/she has insurance or can even afford to pay for such treatment.

I'll tell you one thing for sure. If doctors and hospitals simply stopped treating a person(s) who either didn't have insurance or couldn't prove an ability ot pay for treatment, healthcare legislation would be moving along at warp speed. But of course that won't happen because if the doctors and hospitals acted that way, the lawyers would sue the living bejesus out of them.
