We used
Digitize Home Movies, Photos, Slides & More with Legacybox(R) to convert multiple formats. They send you a box with barcodes. You affix a barcode to each item you send in. They scan in everything and email you regular updates including receipt, intake, processing, completion, and shipping.
They aren't necessarily the cheapest but they did a terrific job, and they regularly run deals so searching for coupon codes can pay off.
