I doubt at 83 years old surgery would be out of the question, but

When I had sciatica 5-6 years ago I had complete relief from an epidural. I was cautioned that I probably would not have complete relief and that any relief would be temporary. My result was the opposite. I had complete immediate relief and no recurrence. Maybe I was lucky.
