As for your question: never in the seven years I’ve been here.
I rode a motorcycle for 25 years or so until I got tired of riding in the rain. Like any vehicle, it was a safe as the driver made it. Golf carts are not inherently unsafe. The only thing that makes them seem that way are the fantastic stories and exaggerations often seen on ToTV.
