Follow the rules, don't falsify data, and the change is enforcement of the rules. if the program is unprofitable for you, change your cost structure, not promise services and then pull them at the last second. The difference between a third world country and societal beneficial capitalism is rules enforcement with personal punishments. Corporate punishments encourages moral hazard, and does not deter third world country behaviors.

Please provide the statistics and cases of these denied claims that are alleged to be prevalent nationwide by United Healthcare. I've seen enough fraud in the healthcare insurance system by the insureds that it would be good to compare the issues.
