Adding your spouse as a fourth in your group when making the reservation/request isn’t necessary (and takes an available slot from someone that might wish to play at the course at that time).
As advised below, simply identify your spouse to the starter as a spectator/observer. The starter will issue a tag for your cart. Be advised that there can be only two people in a cart whether they are golfers or spectators. (I ran afoul of this rule recently when my grandkids were visiting.) Also, spectators are not allowed out of the cart during the round. If you want your spouse to help you read the slope of the green or track your tee shot from behind the teeing ground, then a spectator pass won’t allow that.
