My wife went through the process, in 2005, that was pretty close to what pauld315 described above. But her hospital stay was ten days and one of the other hospitals she could have gone to required 21 days. That was back in CT.

We did ask her doctor when we moved here 3yrs ago where she might have to go if she needed the treatment again and he stated Tampa or Gainesville.
