Gouges, if they're actually cuts in the vinyl flooring surface itself, would have happened at some/any point after the flooring installation. Could have been caused by a number of things: items dropped/dragged on the floor during the remainder of construction, something that happened during moving in, etc.

Bumps (that are underneath the flooring itself) would likely be debris on the floor leftover from construction.......and not cleaned up prior to laying the flooring. Highest likelihood would be small glops of drywall compound that didn't get scraped up off the concrete.

Indentations could be from two different sources, depending on what the indentation looks like. If it's just a small dent/divot in the flooring itself, it is probably due to something being dropped. If it's actually a deeper indentation, where the flooring has actually sunk into a hole, it's likely an imperfection in the concrete floor.
