The $774 price was set by your insurance company. That's why walmart could not tell you the price until they filled it. Your insurance company does that because it wants you to fill it with them. When you bring in a prescription to be filled on your insurance, the pharmacy enters it into the computer. The computer then communicates with your health insurance computer and it is your health insurance computer that sets your copay.
