Does anyone have a clue how you could be electrocuted while charging an I-phone 5?

Just read that Apple is apologizing to a Chinese woman who , unfortunately, was electrocuted** while somehow touching or talking on her I-phone 5 while it was charging............?????? I read further that there were other issues back in 2008. We have an I-phone 4.

Thanks in advance to any techies out there who would like to wager a guess.
I just read about it in the Chicago Tribune.....Monday's issue.
