My guess is the soil bed is way too small it should be as large as the tips of the fronds.

So the lawn fertilizer (which is not good for palms) is hitting the roots.

If the bed was larger and fertilized with Lesco palm fertilizer I think it would not have happened.

Just my opinion.

But call the Ext folks ASAP
