I just did this. If I remember correctly, you can't choose "other" as subject. Fill in one of the other circles, like "site whateveritsays". Now...after you pose your request, it will direct you to another page where you hafta check to be sure the name you are requesting isn't already in use. Then there is a question and answer page...like most commonly asked type of thing. You have got to scroll down thru them all and then check submit again, and your request will go thru. I kept not scrolling down thru the questions and it never got submitted. Took me 3 times to get it right, and then they changed my name right away. Good luck!
