Thanks, Looks like the same form in both places. So many people are told to get an attorney, and in Florida, that is not necessary. After the Terry Shivo experience, we heard an attorney speak. He handed these out to whoever wanted them.

If you can find an Independent Stationery Store, ask for BLUMBERG FORM # 3202, it cost maybe $2 or $3. I filled mine and a reduced copy is in my wallet (just in case). Blumberg's phone number is
