I did a search on TOTV for sliding shelves and came up with this thread which may be helpful. It's from 2013, so I don't know if these installers are still in business. But it's a start.

Affordable Pull-Outs (352) 243-2513. We used them earlier this year, as did several of our neighbors, and were very pleased. Good product, reasonable pricing and very professional.

We used Marla from Custom Drawers (352) 394-6189. Very professional, knew her stuff, came when she said she was coming. Didn't take long. Great investment.
