Dr. Coy's new location is on 441 behind Evolution Fitness and the fitness club is behind a restaurant called Rae's Rae's. The restaurant address is 2468 US Hwy 441/27, Fruitland Park, FL 34731. The address for Evolution is the same and is suite 201. It is just past Spring Lake Drive and before 25A on 441.
