You can have a form, but IMHO, a smart lawyer will be able to piece that piece of paper--never let a renter use your car/ golf cart

It is called a hold harmless agreement. There are hundreds on the web. Typically, one doesn’t apply if a party has acted intentionally or negligently. In fact, many courts have found clauses that include intentional or negligent behavior to be invalid and unenforceable. There are dozens of ways that you could be negligent as a cart owner: maintenance, tire pressure, modification of speed.

Quite frankly if I rented out a home I'd never supply a golf cart to a renter. To me it's not worth it.

Yep, That's why God created insurance. Cover all of your bases if you are renting anything.
