Failed primary & secondary clutch

I own a 2013 Yamaha fuel injected cart. I purchased the cart brand new. It has been serviced every year and I put approximately 2,000 miles a year on the cart. Has anyone else experienced a primary and secondary clutch failure on a golf cart that is less than 6 years old? TIA

I have the same GAS cart, purchased new from the TV with 20k+ miles on it. I've replaced tires, battery, tie rod ends, starter/generator brushes, drive belt, oil and transaxle gear oil. No issues with clutch.
