Can anyone tell me where I can get the necessary paperwork for my doctor to complete so that I can drive my golf cart on the fairway when playing golf because of my medical condition?

Just have your doctor write a letter describing the issue, and take it to the golf management office upstairs in the MVP fitness building in Spanish Springs. They will issue the appropriate ID card, and sell you a "reasonable accommodation tag " for your golf cart. They will also explain the rules for this access because there are some exceptions to your extra access.

Go to golfthevillages.com. Click on golfing in the villages. Then, click on reasonable accomodations. From there, you can download a form that you can fill out and take to your doctor. I tried to provide a link, but it didn't work.

The golf administration offices moved to 1030 Lake Sumter Landing.

I picked one up at the pro shop at Bonifay last week. I think all of the pro shops have them.

If you have a handicap placard from Florida you take the paperwork you have for that and take it to the office. Your doc would have signed off on your need for the placard so not a need to do the other paperwork and the RAT tag eligibility would be for 4 years.

Every executive golf shack has the forms in a file
Just ask for the form from the starter and then have your doctor fill it out for you

Any championship pro shop has the form you can just pick it up

I was told at a starter shack to pick one up at any Country Club Pro shop. I did and filled out the top and dropped it off at my doctors. The form states where the doctor should fax it too. It was pretty easy.

You can go to any golf course or golf shop and get an application. That that to your dr to fill out. You will end up with a ra tag for your golf cart. This year is orange and it is $6.00. That you get at the golf shops.

Any starter at the executive golf courses can give you the form.

You will end up with a tag for your golf cart. This year is orange and it is $6.00. That you get at the golf shops.
Get the handicap permanent designation at any championship golf shop for $6.00.
It's very convenient to wrap around your mirror; and it's good for one year.
