In The Villages the Developer pays both when you close on a new home and when you sell a home you, as the seller, pay them.

It appears that in other communities the buyers pays. Is there a law or is it something that is negotiated?

Also without a mortgage is Title insurance required?

The developer pays for a title search to determine that they are conveying a free and clear title to the property. But, the developer does not buy or pay for an owners title insurance policy. As a buyer, you can buy a title insurance policy, but it is optional. If you don't have title insurance, the developer will still be legally responsible to correct any title issues that may occur in the future, but you may need to sue them to get them to comply. With title insurance, the insurance company will be responsible for correcting mistakes or other title issues.

Aren't all of those things true in the entire State? I was surprised as to who paid what as it is different than our home State.

Lenders title insurance is not needed if no mortgage. Buyer title insurance is optional depending on if you as the buyer wants it.
