I go to design in 2 weeks. We have decided to build the Mossy Oak plan. One area of concern on the Mossy Oak is the smaller "living room." The values make it look big, but the master bed entrance nearly cuts the room in half. The Mossy Oak does cover us on the need for a 3 bath home.
It is our impression that the design team can work wonders to make things to our liking. So to make sure we have done our due diligence has anyone ever taken a 3/2 home and made it a 3/3?

interesting question, I can't answer yes. The master bedroom always seems bigger than necessary, and I would look to reduce the size of the master bedroom in exchange for more living space, but can't be done width wise on the begonia, but lengthwise a bit.
I wonder if someone came in with a custom house plan, based loosely on one of the models, would they just accept it. In fact, if I were to build a new house again, and I might, I would get all layouts ahead of time, and design my own house prior to the design session. . .
Yes, they can do wonders, our neighbors' begonia doesn't look at all like our begonia with the walls all moved around. He can fit a Jeep into his golf cart garage. So although TV put alot of effort into standardization for mass production, the designer also has access to ALL design plans in the historical built database. I would ask the designer directly for what you want to do, and if you have his/her name, i would email his/her ahead of time so she can do the database research. The forum here is limited to maybe a hundred or so active members, and the database holds 100,000+ designs, so the odds that someone before you has had the same thoughts is probably pretty high
Be very proactive and do as much research for what you want to get the big items correct the first time in your design session and that will save you any regret in the future.

I think It depends on the mood of The Developer which depends on how they see the market at the time. Before we came here buyers apparently had a "Street of Dreams" with options. But when we came to buy, the plan was the plan, with few options. A larger lanai, 12' x 29', was offered, so we did that. On the other hand, we couldn't even get them to lay the tile diagonally. Yet, within a year during the deep recession, they were accommodating buyers. During this period an option was to stretch the Lantana two feet lengthwise, which would have made a huge difference in the living area.
I hope you can work something out.

If a formal dining room is not needed, maybe that space can be adapted.
A friend has an Iris with an expended laundry, closet and powder room using that space.
