Very interesting
Question: for a person paying the lowest premium of 104.90, can next years premium increase if they go into a higher income bracket or does the hold harmless rule keep then at 104.90?

Generate less income. It is like going to McDonalds and paying $3.00 for a burger if you make $25K a year and $50.00 for a burger if you make $500K a year.
