15% wouldn't be bad at all (unless you have a $500,000 bill). But I'd get that in WRITING before committing.

Providers who do not accept Medicare rates, but who will bill Medicare, can only charge 15 percent above the Medicare rate. However, there are some providers who "opt out" of Medicare by signing an affidavit. Those providers can charge anything they want to charge, but they must notify patients that they have opted out of Medicare.
