Pet Owners Print And Keep In Your Wallet
 

I am posting a form you can print out and keep in your wallet incase you have to go to the hospital, etc. It lets people know you have pets and who to contact to care for them while you are unable to. I'm not sure how to add this here so that it is printable. Here goes..... You may need to click on the photo and then save it to your computer to be able to print it.

Note: When I printed it the first time it was very large. I reduced it to 33% and tried again. It made it a good wallet size.

Good idea! There are also stickers that you can put on your door or side-light to alert firemen or other emergency responders to pets in your home. Here is an example. This one uses an adhesive, but I've seen others that are magnetic. Could also make your own and print it off.
