Form?
 

Ok, I know I have seen the link before, but I was looking to find the form to send in to get approved for my storm door. Also How long does it take and I just send/fax/etc the form in?

Site Plan???
 

ok, what is a site plan? Can I get long distance?

The site plan is the plat layout they gave you when you purchased (or closed). I made a bunch of 8X10 copies in case I had to send one in and just marked the area in color I wanted to change on the site plan.
