I am posting a form you can print out and keep in your wallet incase you have to go to the hospital, etc. It lets people know you have pets and who to contact to care for them while you are unable to. I'm not sure how to add this here so that it is printable. Here goes..... You may need to click on the photo and then save it to your computer to be able to print it.

Note: When I printed it the first time it was very large. I reduced it to 33% and tried again. It made it a good wallet size.
