OK, now I understand. It is not a supplement but rather a Medicare Advantage HMO (if it were a supplement, you would pay virtually no medical costs but would pay a high premium). I also have a Medicare Advantage Plan through United Healthcare, but it is a PPO, not an HMO (referrals not required and also no premium).
