Not the actual website to do survey, but the letter to inform you what the website is. Find all info blank in each area on letter. Would not be able to go to survey site with out that info on the letter. It's blank in three areas of my letter. Maybe I have asked for the moon in the past and they are sending me the letter with out www. LOL. Strange that both of my survey letter are missing that info
