I would insist on a blood test if that doesn't happen then find another rheumatologist. Gout is in the rheumatoid arthritis family and can be very serious. Diet (I believe) can contribute to your symptoms. That being said, if it is gout you need to be proactive in it's treatment. Discussing this on a "social network" site...asking for thoughts on symptoms won't be helpful, though you might be able to find a way towards a practioner. Best wishes in your quest!!!!
