Default Pavers have been flaking

We got paver stones put in and since day one we noticed flaking and it has going on two years and we have brought this up with company and they will replace but will this be on going? R these defects? Can u look at pics and let me know thanks

Are they driveway pavers? 2 plus inches thick?

Perhaps the amount of cement and sand that was used was not a correct mix.

That's only my "guess."

This looks like fractures due to movement, is this where the cars drive or is this in another location? Also, is there sand below and between the pavers? I am no expert, but this installation may not be up to standards. You may want to contact an expert in this area for an opinion.
