I think you are supposed to call ALLSUP to fill out that form. Is it about your past work or your daily activities? We are real busy here these last two days with our grand kids. A good day for me would be Saturday while my husband is driving us home to FL. Do you have a cover letter that says they sent a copy to ALLSUP or is there a cc: on the letter you got? I don't want to fill out this form and have them call you to get another one.
