It is made by Mohawk. I would check with Lowes or Home Depot to see if they carry that particular tile. If not, you can contact an authorized Mohawk Dealer.

Great Lakes Carpet has a store in Wildwood and Carpet One has a store in Spanish Springs.

If the tile is out of production do an internet search for out of production floor tiles, in the past I found several sites where you might get lucky and find some.

Look carefully for the same lot number. The coloring of the tile will be slightly different with each batch made by the manufacturer. 11 years ago? Might be difficult to perfectly match.
