Chiropractor/sport medicine w/physical therapist

Does anyone know of a chiropractor that does sports medicine and has a physical therapist at his facility/clinic? I haven't been about to find a pain/rehab center like that down here. You only get 10 chiro visits (BCBS), 20 chiro visits (Medicare) but 75 physical therapy visits. Thanks.
