Agree, as the form has nothing to do with your appointment and when you get to the appointment they ask you if you’d like to be screened for various cancers via a blood test that my doctor didn’t order. They also want you to sign to pay for the test as they aren’t sure if you’re insurance will cover it or not??? Funny how they knew my mammogram would be covered by my insurance but aren’t sure about this? I strongly disagree with duping people into thinking that you need to fill out the form before your appt, and pushing a test for an unknown amount of money (up to $250) that your doctor didn’t order!

I too got this form. After asking questions I declined to fill it out. Too much data mining going on everywhere and I don't trust where all this info is going.

I got it too and declined. There is a history in my family but my question would be..."so one tests positive, then what?" There isn't a preventative that I know of other than trying to do the right thing with diet, exercise etc and that doesn't necessarily mean one will stave it off. "Knowing" would just add to stress, IMHO. My philosophy is "IF" I get it (or any disease) we'll deal with it as effectively as possible.

By federal law you never have to give your social security number to anybody. So don't, and if they complain walk out and find another place

Telemarketing and yes reselling but the way that test is now about 18,000 my Michigan blue cross paid for it however guess what ? Having that test only decreases a reup by 7% even that number is reliable

It's a genetic test. I did it recently with AMBRY GEN. If you are at high risk for developing breast or ovarian cancer (based on yours or family history), your insurance should pay for it. Mine did. Call your insurance to be sure.
