Do you ever read the notes after a doctor visit. I read all mine, and I cannot believe the things they print out that supposedly occurred at the visit. For instance, abdomen soft,, non-tender. Never touched abdomen. No Carotid bruit, never checked carotids. And on and on.... I am an RN..... I know what they checked and didn't check.... it is unbelievable what they document. And the list of diagnoses is never correct, ie, DM... I am not diabetic, HTN, I never had high BP and never medicated for same. My spouse's reports are the same. One doc had noted in the H & P (pt history) that my husband has 5-9 drinks of alcohol daily..... NEVER in 40 years have I seen my spouse have more than 1 cocktail before dinner.... it's deplorable.....and this is not only in TV....

I blame the Electronic Medical Records that are now pretty much required... If you don't have copious amounts of garbage in your notes, the insurance companies deny payment...

I'm dealing with $8K worth of denied claims from Medicare right now. They said it's because my notes didn't include things like "The patient tolerated the procedure well"...

Total insanity...

No one writes notes anymore, they just "check off boxes"...

This happens everywhere. I lived in Boston and had a shoulder issue. I saw an orthopedic surgeon in my primary care doc’s building and he wanted to do surgery for a torn labrum. I was advised by an acquaintance in the medical profession to seek a second opinion. I went to a sports medicine facility and got a completely different diagnosis. Turns out that I did need surgery on one shoulder but for a completely different problem and PT on the other shoulder. I later found out that my original doctor had several malpractice lawsuits pending against him and was eventually let go by the practice. He moved to another practice and is still practicing.
