I'm dealing with $8K worth of denied claims from Medicare right now. They said it's because my notes didn't include things like "The patient tolerated the procedure well"...

Total insanity...

No one writes notes anymore, they just "check off boxes"...

This happens everywhere. I lived in Boston and had a shoulder issue. I saw an orthopedic surgeon in my primary care doc’s building and he wanted to do surgery for a torn labrum. I was advised by an acquaintance in the medical profession to seek a second opinion. I went to a sports medicine facility and got a completely different diagnosis. Turns out that I did need surgery on one shoulder but for a completely different problem and PT on the other shoulder. I later found out that my original doctor had several malpractice lawsuits pending against him and was eventually let go by the practice. He moved to another practice and is still practicing.
